Horizon Healthcare will be holding a Summer Physical Clinic on July 6th from 9:00 a.m. - 3:00 p.m. at the K-12 in Albion. Please see additional information below.
-This is a come and go, no appointment necessary clinic.
-A parent must accompany the student to the clinic.
-It is free of charge, however, they will bill insurance if applicable.
-Parents & students should use the Jr. High entrance. (Door #1 on the west side)
-Registration will be in the Jr. High hallway by the trophy case.
-Students may have a required and/or sports physical. Physicals are required for PK, K, 6th, & 9th grades.
-Physicals are also required for students to participate in tryouts for a sport.
-A Sports physical is valid for one year and 30 days from the date it was completed.
-A Required physical is valid for one year from the date it was completed.
-A sports physical cannot expire during the sport season they are participating in.
-This clinic is early enough for students playing Jr. High softball & baseball, or high school volleyball & football to get their physicals before tryouts.
